1. Incorporate Massage Responsibilities

Incorporate Massage reserves the right to refuse services to any individual(s). Requests for sexual activity of any kind will not be tolerated, will be viewed as solicitation, and reported to the proper authorities. The services will not be rescheduled if this occurs. Sexual interaction or discussion of any kind between the client, participant and the practitioner is not appropriate. The practitioner reserves the right to refuse to discuss any topic which they feel is inappropriate. Any discussion between the practitioner and the client is completely confidential. 

Massage therapists can adjust pressure, musical volume, work longer on a specific body area, if requested. The client may choose to refuse any onsite massage methods, stop massage at any time, and is free to leave.

The client or participant may or may not choose to talk. The massage therapist will listen and share professional expertise. It is the responsibility of the client to keep the practitioner informed of any medical treatment currently being taken and to provide written permission from the physician, chiropractor, physical therapist, etc., that the onsite massage or virtual wellness services may be continued. The client must also keep the massage therapist or other relevant wellness or mental health practitioner informed of any changes in health conditions.

 

2. Client Responsibilities

a.     Clients shall provide access to the designated facility, as well as a suitable facility with dimensions of 5’ by 5’, which include a garbage can.

b.    Client is directly responsible to arrange parking for Incorporate Massage personnel that come onsite to perform Services.

c.     All client personnel must arrive at the designated massage area five (5) minutes prior to the scheduled starting time of the session. If late arrival is inevitable, the service(s) for that personnel will be shortened in order to keep on schedule. No full or partial refunds will be given.

3. Financial Responsibilities 

Client shall pay Incorporate Massage the charges as detailed in the relevant Statement of Work for the Services provided by Incorporate Massage under such Statement of Work. Payments for services are due at the time of booking the services. A valid credit card is required to be provided in order to book your services, no matter the payment method used to purchase the massage services. If paying by check, checks must be received at least 48 hours prior to your scheduled event in order to receive services. If paying by ACH draft, the draft must be received at least 48 hours prior to your scheduled services.

Client shall receive an invoice from Incorporate Massage at the date of services. Invoices are due and payable immediately upon receipt. 

Late payments for events are subject to a $50 late fee after the first 24 hours, and a $50 fee for every 30 days thereafter. If payment falls delinquent, Incorporate Massage may also suspend provisions of the Agreement or SOW until such delinquency is cured.

a. Subscription Late Fees. Late payments for clients with a subscription Service are subject to a $250 late fee after the first 24 hours, and a $250 fee for every 30 days thereafter.

b. Returned Check Fees. The fee for returned checks will be a $100 additional fee applied to your outstanding invoice.

The fees accumulated are exclusive of taxes and other similar charges as may be required under applicable State and Federal laws. Client shall be responsible for, and shall pay or reimburse Incorporate Massage for, all taxes, duties, and like charges arising.

 

4. Client Cancellation

As a corporate client, you must provide at least three (3) business days of notice if a reschedule, cancellation, or modification to the services is needed. If cancellation or modification to the services is given more than 24 hours’ notice, a 50% of the services fees will be charged to the credit card provided.

Full refunds will not be given.

Cancellations or appointment modifications with less than 24 hours’ notice will be charged the full fee for services. No refunds will be provided for deposits or cancellations received less than 24 hours before a scheduled appointment. Additionally, events with four (4) or more Incorporate Massage personnel practitioners will be charged a 15% non-refundable fee for any cancellation or reduction in hours, no matter how much notice is given. 

5. Inclement Weather Policy

If an event is canceled by the client for inclement weather, no fee will be charged for the event or cancellation if;  1) the weather event meets the National Weather Service definition of a “Dangerous Weather Event,” and 2) the client reschedules the event for a future date within 12 months. To qualify for having the fee waived, NOAA & The National Weather Service should have issued a Major Storm Warning either during the day of the event or in advance of an expected event.

Incorporate Massage reserves the right to cancel an event in the case of inclement weather if it jeopardizes the safety of our practitioners.
